This session takes a deep dive into the Home Health Value-Based Purchasing (VBP) model using 2025 national performance data, with a focused analysis of agency performance across New England. Participants will explore regional trends, performance variation, and key drivers of success and opportunity. We will also highlight best practices emerging from the National Patient and Family Satisfaction Quality Improvement Project, translating data insights into practical strategies agencies can use to improve outcomes, patient experience, and VBP performance.
